I'm sure there has to be some genetic factor in play, imagine how much it must cost to feed that child.

Child protective services should have come in way before this, the child is clearly not having her needs met if she's at that weight and still gaining. Imagine the social and mental implications of having that weight, you know she isn't accepted by other children, and has to have horrible self images, I mean, wow, that's got to scar you for life, even if she does manage to take it off. Her bones aren't developing normally with all that extra bulk.

Regardless though, even if she has some sort of pituitary disorder or thyroid problem causing her excessive weight, it takes years to develop 400lbs. The parents could be helping her to control her weight or specially designing one of those toddler walker-majigs for her so she can at least move her legs.
